If you are registering on behalf of a care home, support service, school, community group or other organisation, please complete this form before booking your first Wheels for Wellbeing cycling session.

The information you provide helps us understand your organisation, who may attend with you and the support arrangements you have in place, so we can help everyone take part safely and enjoyably.

Before completing the form, please make sure you are authorised to register on behalf of your organisation.

Organisations attending our sessions are responsible for providing appropriate carers, support workers or other personal support for the people they bring. Wheels for Wellbeing staff provide cycling instruction and session support, but we do not provide one-to-one personal care or supervision.

By registering, your organisation will be asked to agree to our Cycling Sessions Terms and Conditions and Cancellation and Refund Policy. Please make sure relevant staff and support workers are familiar with these before attending a session.

We will use the information you provide in accordance with our Privacy Notice.

Participant fees are £7 per session and should normally be paid by card when booking online. Payment by invoice is not normally available. In exceptional circumstances, we may agree to invoice an organisation instead. If your organisation cannot pay by card and needs to request payment by invoice, please contact us before completing this registration form. Invoice arrangements must be agreed with Wheels for Wellbeing in advance.

Please use an email address that can be accessed by more than one authorised member of your organisation, where possible, for example info@carehome.com rather than an individual staff email. This helps prevent access problems if the usual account holder is away, leaves the organisation or is unavailable. This email address will be linked to the organisation's Wheels for Wellbeing online account and used for account access and booking-related communications.

What your organisation is responsible for:

Please make sure your staff, carers and support workers understand and follow the responsibilities below when attending Wheels for Wellbeing sessions.

We will make sure all staff, carers and support workers attending sessions are registered with Wheels for Wellbeing where required and are familiar with the relevant session guidance.
We will make sure all participants attending with our organisation are correctly registered before taking part.
We will provide enough staff, carers or support workers to meet the needs of the people we bring to sessions.
We will remain responsible for providing any personal care, supervision and behavioural support required by the people we support.
We will give Wheels for Wellbeing accurate and relevant information about support needs, known risks, behaviours or triggers that may affect safe participation.
We will tell Wheels for Wellbeing if there are any important changes to a participant’s needs or support arrangements before attending.
We will cooperate with Wheels for Wellbeing staff and follow all safety instructions during sessions.
We understand that Wheels for Wellbeing may pause or stop someone’s participation if there is a significant safety concern, and may ask for an individual risk assessment or support plan before they return.
We will make sure our staff and the people we support are aware that bookings are only valid for the specific session time booked.
